Child Care Subsidy Terms and Conditions

This form has a list of terms and conditions for parent(s)/caregiver(s) to agree to in order to receive child care subsidy, including a request to share personal information with other program areas or service providers in order to coordinate your child care subsidy services.

Terms and conditions for receiving child care subsidy

  • Eligibility for subsidized child care shall be governed by the criteria as set out in the S.8-12, O. Reg. 138/15; Child Care and Early Years Act, 2014, S. O. 2014, c. 11, Sched. 1, as amended from time to time or replaced by successor legislation, as well as criteria as set by the Region, including but not limited to, the resolution of the Regional Council made on August 16, 1990 (“the criteria”).
  • The criteria may be amended or changed from time to time by the Province of Ontario or the Region, and shall be binding on you without the requirement for written notice to you of the change.
  • The Applicant acknowledges and agrees that the criteria have been explained to them by the Region.The Applicant agrees to provide the most recent Federal Notice of Assessment or Reassessment or Canada Child Tax Benefit Notice.
  • The Applicant shall advise the Region of any change to eligibility pertaining to the criteria, which includes but is not limited to marital status, family composition, living arrangements, employment, education, or Ontario Works/Ontario Disability Support Program status within seven (7) days of any such change.
  • Eligibility for child care subsidy will be granted for a maximum of one year and will be reviewed on an annual basis.
  • If the Region determines that the Applicant received subsidized child care when ineligible pursuant to the criteria, then the Applicant, the Region may immediately terminate the Applicant’s Child Care Subsidy and the Applicant, upon written demand from the Region, shall repay the Region for the subsidy paid during the period of ineligibility (“the subsidy overpayment”) and the Applicant acknowledges and agrees that the Region may proceed with a claim before a court of competent jurisdiction to recover any Subsidy Overpayment thereon.
  • All of the statements in the foregoing application are true to the best of my/our knowledge and belief and no information required to be given has been concealed or omitted.

Consent to exchange personal information

As needed, Child Care Subsidy staff at the Regional Municipality of Waterloo will exchange information about me/us to staff of Licensed Early Learning and Child Care, Extended Day, Youth Development, or Recreation programs in order to coordinate my/our child care subsidy services.

Acknowledgement

By electronically signing below, I/we ("the applicant(s)") acknowledge and agree:

  • To be bound by the terms and conditions above if this application for subsidized child care is accepted by The Regional Municipality of Waterloo ("the Region"); and
  • To authorize the Child Care Subsidy staff of the Region to exchange personal information about me to staff of Licensed Early Learning and Child Care, Extended Day, Youth Development, or Recreation programs to assist in the ongoing administration of my/our child care subsidy.
