Reporting Form for Temporary Foreign Workers Isolating at Hotels

Region of Waterloo Public Health (ROWPH) currently has a Class Section 22 Order in place asking for the contact information for any employers of migrant workers if the workers are living or working in the Region (https://www.regionofwaterloo.ca/en/health-and-wellness/resources/Documents/MigrantWorkersSection22.pdf ).  This applies to employers who are not located within Waterloo, if their employees are staying or working within the region for any reason.  This form is NOT to be used to collect information on the workers, just their employers contact information.  Public Health will then contact employers to gather any further information we may require. 

This information will assist ROWPH conduct more rapid case management of any worker who happens to test positive in a hotel, or in any potential outbreak scenarios, should it be necessary. All information collected will be kept confidential and only be used by public health staff for covid case management.
