Special Event Form for Vendors

All special event organizers and vendors who sell or give away food must submit a special event notification form to Region of Waterloo Public Health.

Please note that events/vendors that only sell/give away store bought, pre-packaged, non-hazardous foods such as candy, chocolates bars, granola bars and chips are not required to submit a special event application form. If in doubt as to whether a form should be submitted, please submit the form and Public Health can assess the food item and event.

Please complete and submit this form at least 30 days prior to the event. Notifications may not be accepted/reviewed after this time.

Mobile vendors already licenced by the local municipality and inspected by Region of Waterloo Public Health are not required to submit a Special Event Vendor Form.
